Request for Catastrophic Illness time transfer

Paul Chin has sent the Executive Board a request that we would like to pass along to you.  Paul is an AFA past President and a founding member.  His leadership has shaped many of the values that the AFA holds today.  Please read the following excerpt from Paul’s letter to the Board:

I am reaching out to you for help, not so much for me but for my brother Ted who has been diagnosed with liver cancer.  Currently he is undergoing treatment and is on the waiting list for a transplant.  He is a city employee with Muni and because of the long term treatment and the unknown wait for the transplant, he is exhausting his sick and vacation time. He is currently registered under the "catastrophic sick time" program.  I need your help in requesting members of the AFA to donate sick time to his account. His account number is 001455.

Sincerely

                Paul

You can click the link below to download the Donor Form.  You can also obtain a form from any City HR dept or Payroll dept.  Ted Chin’s Recipient Identification Number is 001455.

Thank you from the Executive Board.
